If you child has asthma and will use an inhaler during school, practice or sporting events, then this form must be filled out by their physician. The medication form must be filled out for any medication taken at school. (This includes pain relievers such as Tylenol and Ibuprofen) Students cannot self-medicate. All medication must be given by the school nurse. This includes prescription and any over the counter medication. The form must be signed by a physician.
